Dear Mater Dei Prep Parents,

 

Mater Dei Prep uses an online program called FamilyID to register students for sports participation at the school. As the parent/guardian (Account Owner), you will create ONLY one personal profile for your child that makes subsequent registrations seamless. Do not create multiple logins/personal profiles.

 

In order to participate in a Spring Sport at Mater Dei Prep, a parent/guardian (Account Owner) MUST register on www.familyid.com.

 

To be approved to play a sport at Mater Dei Prep this winter:

1. Students MUST provide to school one hard copy of the completed Pre-Participation Physical Evaluation Form for approval

2. Parents/Guardians must register their son/daughter on FamilyID

 

The Pre-Participation Physical Evaluation Forms can be found under ATHLETICS in ATHLETIC FORMS on the Mater Dei Prep website at www.materdeiprep.org. Hard copies of the Pre-Participation Physical Examination Form are also available at the front desk in the Main Office of Mater Dei Prep.

 

Pre-Participation Physical Evaluations are valid for 365 days from the date of completion. Questions regarding your child’s Pre-Participation Physical Evaluation, contact the main office or school nurse at Mater Dei Prep.

 

All hard copies of the completed Pre-Participation Physical Evaluation Exam MUST be handed into the MAIN OFFICE at Mater Dei Prep
