Mater Dei will release student and alumni transcripts as requested. In order to have a Mater Dei transcript sent to support college and scholarship applications, student internship applications, etc., please note the following:

Transcript Release Form

Before Mater Dei can release your transcripts to any college, scholarship review committee, or prospective employer, a Transcript Release Form is required. This form gives Mater Dei permission to furnish your academic records to outside parties and must be on file in Student Services before we will release this information.

Transcript Request Form

In order to have your transcript sent to specific colleges, scholarship review committees, or prospective employers, you must submit a Transcript Request Form for each transcript that you want sent.

Please make as many copies of the release form as you need. Copies are also available in the Student Services Office.

Allow five school days for completion of your request. Requests for transcripts are processed in the order they are received.

When completing your Transcript Request Form, please note that you are responsible for indicating the Application Classification (Early Decision, Rolling, etc.) and the required due dates for receipt of the transcript at the college or university

If your transcript requires special handling, please clearly indicate that specific information on the Transcript Request Form (Note: some scholarship applications will be returned if the application and supporting credentials are not mailed according to the directions).

You are also responsible for forwarding your SAT and/or ACT scores directly to the college or university through The College Board and The ACT Office. Student Services will send copies of the scores provided to Mater Dei, but many schools require that the test center furnish this information to them directly.
